That looks good Tyler, my only concern is specifying 75% GW or FW for conversions - thats one of the main benefits of playing in an FLGS is not having to do that. Fine for GW stores who need to be able to sell what players see on the table, but that's not the case at Dojo - they sell all kinds of models for all kinds of games. As an Ork player, this is the reason I don't play in GW tournaments. Well that, and I am awful at warhammering. And I haven't painted anything......and some of my Orks are actually just old Monsters In My Pocket figurines glued to bases....

Also maybe put in a rule regarding TO having final say on what is deemed a proxy and what is deemed a conversion, or make them email you and ask you first. When I planned that tournament back in April before my daughter was born the day of the tournament, I had everyone email me their lists ahead of time so I could A) make sure they added up and B) make sure they weren't being too dickish. Its only a short step from there to have them also include a list of any conversions they plan on using for approval. Most GTs have the same set up - email your list ahead of time, and clear any questionable conversions or proxies ahead of time.

I do not think it would be at all unreasonable to ask people to submit any major conversions for approval. I mean half my army is converted to some degree, but I understand that something like my scratch built Skorchas are a different kind of conversion compared to some converted Tank Bustas. I take pride in trying to fit any conversions to the same footprint and height of whatever I am mimicking, but I can see how not having limitations on that could get out of hand fairly quickly. I would honestly be slightly more concerned with WYSIWYG, because for fluff purposes (I play Death Skullz) I have attached ALOT of superfluous parts onto pretty much anything bigger than an ork boy.....many of which serve absolutely no discernible purpose other than to look cool. I have a Battle Wagon sporting non-functioning dreadnought arms and that son of a bitch looks spectacular.

As soon as I get the final detail work done, and a base coat on it. It's open top, so I am trying to make the back bed look more interesting.
